Distinguished Service Award

The highest honor that a club or district can bestow through Mid-South Lions Sight and Hearing Service is named for the organization's first president, Edwin Dalstrom. Lions Clubs, Districts or Multiple Districts can honor a club member or a member of their community with this prestigious award by making a $1,000 contribution to Mid-South. To name a Dalstrom simply download the application by clicking the Dalstrom brochure below, fill it out, and email OR mail to our office. If your club/district/MD has already donated $1,000 to Mid-South during the fiscal year for which you are seeking the award, no additional contribution is required. If not, please send a check for $1,000 with the Dalstrom form.

PROGRESSIVE DALSTROMS are also available to those who have already been honored with an award. The progressive award recipients receive a pin with diamonds corresponding to the number of awards received. (Second Dalstrom, pin with one diamond; Third, pin with two diamonds). PERSONS RECEIVING MORE THAN 3 DALSTROM AWARDS WILL RECEIVE ANOTHER ORIGINAL PIN FOR EACH ONE THEREAFTER.

$1,000 DALSTROM AWARD CONTRIBUTIONS MUST BE MADE WITHIN THE CURRENT LIONS YEAR. WE CANNOT ACCUMMULATE DONATIONS OVER A LONGER PERIOD OF TIME. WE APOLOGIZE FOR ANY INCONVENIENCE.
